My Lords and Gentlemen, The Annual Report, consisting of 53 weeks, ended April 1st, 1871, during which period 2343 births and 2049 deaths were registered in the Parish, including those of 266 non-parishioners in St. George's Hospital, 103 in the Workhouse, Mount Street; 67 in Little Chelsea Workhouse; 2 in the Belgrave Hospital for children; 1 in the Orthopœdic Hospital, Oxford Street, and 41 in the Home for Deserted Mothers and Infant?, No. 3, Cumberland Street. The total mortality (2049 shows an increase of 212 deaths, as compared with that) 1870, and gives an annual death-rate of 23 per thousand of the population. But deducting the deaths (266) of non-parishioners at the 2 preceding hospitals excepting "The Home," the death-rate at home was 20 per 1000. This calculation has been made according to the census of 1861, when the population of the parish was estimated at 87,747. But through the kindness of Mr. Clode, Secretary of the Registrar General's Department, I have received the following Census of this Parish up to 1871, the figures of which are at present provisional, as the returns have to undergo a strict examination before the final numbers are determined:— CENSUS, 1871. Population. Hanover Square Sub-District 18,700 May Fair „ „ 13,020 Belgrave „ „ 58,043 Total of the Parish of St. George, Hanover Square 89,763 Hence it appears that Hanover Square has lost 1,070 inhabitants, but that May Fair has received 372, and Belgravia 2,944 in addition. Adopting this Census for that of 1861, we find that the death-rates would be 22 and 19 per 1,000 respectively, instead of 23 and 20. It should, however, be noted that the mortality admits of still further reduction, owing to the establishment of a "Home for Deserted Mothers and Infants," No. 3, Cumberland Street, Pimlico, where a large majority of the deaths have occurred in non-parishioners. 132 persons were admitted, of whom 41, or about 31 per cent. died, and of these only 8 were parishioners. The following statement gives the number of deaths for the four quarters of the year, amounting to 487 in the first quarter, ending June 25th, 1870, 444 in the second; 539 in the third, and 578 in the fourth, ending April, 1871. 3 393 infants, or about one-fifth of the total mortality, died under one year of age. New Industrial Dwellings. The Coleshill Buildings in Ebury Street and the Pimlico Road, containing 120 tenements, and the Ebury Buildings, in Ebury Square, holding 72 tenements, were opened on Friday, November 18th, 1870, by the Directors of the Improved Industrial Dwellings Company, who have conducted a variety of experiments for the purpose of securing a constant water supply for their tenants, and having, as they believe, succeeded, the perfected arrangements have been fixed for the first time in these Buildings. The " New Models" situate in Grosvenor Mews, New Bond Street, are provided for 20 families—10 single rooms and 10 double rooms, including a constant water supply. Zymotic and other Diseases. The mortality from Zymotic disease amounted to 479 against 319 of the previous year, and includes, from Smallpox, 32; Measles, 29, against 17; Scarlet Fever, 137, against 64; Diphtheria, 4 against 8; Quinsy, 2; Croup, 12 against 9; Whooping Cough, 50; Typhus Fever, 16 against 43; Erysipelas, 24 against 9; Enteric Fever, 32; Pyasmia, 27 against 18; Carbuncle, 2; Dysentery, 4; Diarrhœa, 79; Cholera, 2; Remittent Fever, 1; Rheumatic Fever, 2; Rheumatism, 1; Syphilis, 12 against 4; Thrush, 4; Metria, 1. Tabes and Scrofula proved fatal to 22; Consumption, 232 against 247; Lung Disease, 321 against 320; Hydrocephalus, Convulsions, and Teething, 98. In the quarter ending September 24th, 1870, the highest number of Deaths from Zymotic disease occurred, the total being 157; the cause being attributable to Scarlet D 4 Fever, which numbered 51 victims, 35 of whom were under 5 years of age. The deaths from accident or negligence, numbered 60 against 68 last year, 43 of which occurred to nonparishioners in St. George's Hospital. Of the parishioners, a licensed victualler, aged 54, died from fractured cervical vertebras, at "The William IV.," Commercial Road, Pimlico. Another, aged 45, died from the same cause, having fallen from the roof of No. 5, South Street, Grosvenor Square, when beating carpets. A labourer, aged 69, died from exhaustion from a shock after fractured ribs, caused by falling against a chair on which he intended to sit, at No. 3, Commercial Road. The son of a porter, aged 7 years, died at the Orthopaedic Hospital, Oxford Street, from fractured skull and laceration of brain, caused by falling over the baluster of the stairs on which he was sliding when at play. The son of a cab-driver, aged 6 years, died from injuries to his head and body, by a street cab falling over upon him when seated on the box, the horse running away without the driver, who had left the box to pick up a fallen rug at the corner of Warwick Street. A labourer, living at No. 9, Ranelagh Road, aged 38, died from fracture of the skull, produced by being pulled over by the handle of a crane when hoisting a stone with insufficient purchase, his head striking the ground, at Steward's Wharf, Pimlico. Another, aged 75, died in the Workhouse, Mount Street, from Erysipelas in his head and face, after a scalp wound from falling down stairs in the Workhouse. An infant, aged 9 months, was found with her night-gown on fire in her parent's room, at No. 5, Brown Street, Grosvenor Square. Another, aged 16 months, had its abdomen and thigh scalded by a pot of boiling coffee on the table in his parent's room falling on him, at No. 3, Shepherd's Market; and a boy, aged 3 5 years, died in St. George's Hospital, from bum of face, throat and arms, having been found with his clothes on fire in his mother's room, at No. 8, Passmore Street. A carpenter, aged 40, died from spasm of the epiglottis, by a piece of bread in the trachea. The son of a labourer, aged 13, was drowned in the "lie bye," Grosvenor Row. An infant, aged four months, was suffocated when in bed with its mother, at No. 3, Cumberland Street. A newlyborn infant was found with a ligature placed round its neck near the ''Magazine," Hyde Park. Of the suicides, a gentleman, aged 40, threw himself from a window at the Albemarle Hotel, Albemarle Street, when insane, and died in a cab near St. George's Hospital, from fractured ribs, blade bone, and other injuries. A man servant, at No. 22, Berwick Street, aged 33, poisoned himself by morphia. A coachman, aged 56, wounded himself in the throat, at No. 19, North Bruton Mews. A gardener, aged 48, wounded himself in the throat with a knife, at No. 57, Gillingham Street. A woman, aged 45, drowned herself in the Serpentine, Hyde Park. A nursery governess, aged 33, hung herself by a small rope to a peg in the wash-house. The wife of a sub-inspector of government, aged 41, hung herself by a scarf to a bed post. Of the 16 deaths from causes not specified or ill-defined, were those of 9 infants, "newly born," found in the Grosvenor Road Dock; on the door step in Warwick Street; in the garden of No. 257, Vauxhall Bridge Road; Shepherd's Court, Upper Brook Street; in the dust hole at No. 27, Eaton Place; near the Police Station, Hyde Park; in the pan of a water closet at Victoria Railway Station, and 2 in Hyde Park. Sickness. In presenting this subject, I have to acknowledge the voluntary assistance rendered to me by Dr. Webb, Messrs. E. .Jay, C. P. Ward, C. Connock, and S. R. Phillips. The number of cases relieved by the Parochial and d 2 6 Dispensary Medical Officers in the Parish was 36,904. Of these, 21,915 were patients at St. George's Hospital. It appears that the whole includes 59 of Small-pox; 49 of Chicken-pox; 194 of Measles; 277 of Scarlet Fever; 109 of Whooping Cough; S of Croup; 594 of Diarrhoea; 1 of Dysentery; 87 of Continued Fever; 88 of Rheumatic Fever; 28 of Enteric Fever; 65 of Erysipelas; 5 of Diphtheria; 4 of Ague, and 5 of Carbuncle. There were 1,998 cases of Bronchitis; 39 of Pleurisy, and 43 of Pneumonia. Vaccination in the Hanover and May-fair Sub-Districts. Above 1 year 3043 Above 1 year 177 Under „ 248 Under „ 2 3,764 181 7 Two hundred and eighty-five inmates of St. George's Workhouse were re-vaccinated by Mr. Jay, without subsequent inspection of the results. Also during the last quarter there were 997 cases; 930 were successful. Of these, 172 were Primary, and 758 were Secondary Cases, as follows:— Primary Cases 173 Secondary Cases 824 Successful 172 Unsuccessful 1 173 Successful 758 Unsuccessful 35 Not seen 32 824 Small Pox. This dreadful disease, which has been a scourge in the Parish and elsewhere, first made its appearance at Lansdowne House, about June 14th, 1870, when a Governess, aged 50, vaccinated in infancy, arrived from Paris, and went to bed immediately, became ill with small-pox and died on June 24th. It did not spread to the servants, all of whom were re-vaccinated, while other preventive means were adopted. About the middle of October an infant aged one year was attacked with small-pox at No. 20, Eaton Mews South, after vaccination performed a fortnight before, and recovered. Another patient, aged 29, vaccinated, was removed from No. 1, Park-place, St. James's-street, to the Hospital in November. A young man, unvaccinated, had confluent small-pox on November 19th, at No. 114, Warwick-street, whence he was taken to the Hospital, and a child was affected at No. 18, Hobartplace. Two women, named Esther Masser and Sarah Collis, suffered from the disease when in-patients at St. George's Hospital. The former had been a patient there seven weeks before with an attack of confluent small-pox, and occupied a bed in the Holland Ward, which is used for 8 physicians' cases, but the latter had never left her bed for five months in Drummond, a surgical ward, owing to a bad leg, yet she had modified small-pox. Upon the appearance of these cases I sent an inspector to the parochial schools, urging the necessity of a careful examination of the pupils' arms, and hand-bills were circulated, strongly recommending parents to have their children vaccinated. Then five cases were reported in December, one at No. 8, Berwick-street, Pimlico, another at No. 118, Buckingham Palace-road, a third at Chesterfield House, South Audley-street, a fourth at No. 1, Mount-row, Davies-street, and the fifth at No. 10, Pollen-street. So far a history has been given of the outbreak of small-pox and the course which it took. Whenever a patient was sent to the Hospital the room was disinfected with sulphur and cleansed, the bedding burnt or disinfected, the walls stripped and limewhited. The epidemic having assumed greater dimensions in January, 1871, the Committee of Works resolved unanimously that I should be empowered to use any means that might be considered necessary without my coming for its sanction previously. Early information of all the cases of small-pox was then obtained, which were removed when practicable to special hospitals. The houses and things were disinfected with as little delay as possible, overcrowding was abated, dead bodies removed and compensation paid for bedding and other things which had been destroyed. A disinfecting apparatus was placed in a room constructed for the purpose on the Parish wharf, Ebury Bridge, and a covered spring truck provided for conveying infected bedding. I appointed two assistants, one for the in-wards, the other for the out-wards, to make a house to house visitation in infected streets' or courts, to promote vaccination or re-vaccination, and to 9 abate nuisances. I also suggested to the Committee the propriety of the Vestry taking steps to provide a mortuary. Each of the district vaccinators appointed an additional day in the week for vaccinating or re-vaccinating the poor in the in-wards and the out-wards. Hand bills, recommending the immediate removal of manure were posted in all the Mews, and others, containing the best measures for preventing Zymotic diseases, were sent to every house in the Parish. In March the disease began to extend to some of the upper classes and was spreading throughout the Parish. 186 cases were recorded, and many others no doubt were wilfully concealed, which greatly increased the risk of contagion. March 20th, „ No. 10, Clayton's-yard, m., aged 5 years, vaccinated. 11 Sanitary Improvement. The number of cases entered in the complaints book and investigated was 1,455; verbal notices given and orders served, 163; orders by Committee to summon, 6; summonses taken out, 2; magistrate's orders, 1; slaughterhouses, 20; cowsheds, 8; marine store dealers, 25; bakehouses, 80. The last four are frequently inspected as to their cleanliness, and the removal of refuse, and are limewhited when necessary. Workshops' Regulation Act. Twenty-seven establishments were visited, against fortytwo in the previous year and fifty-eight in 1869, showing a great decrease of complaints, although a most careful supervision has been exercised. It is satisfactory to find that the Act is much better observed in the Parish than when it was first passed, and in some instances where complaints have been made the dressmakers willingly received a half-holiday, as an equivalent from the employers whenever they occasionally worked beyond the legal hours with the consent of both parties. Nevertheless victims of the needle are still offered upon the altars of fashion, as proved by a young person who had been kept at work on June 15th, from nine in the morning to one o'clock the following morning, and whose employer I summoned. I have endeavoured to carry out the Workshops Act by the abatement of over-crowding, by enforcing due ventilation, and closing at the legal time, so as to prevent the scandal and suffering of dressmakers still being compelled to toil for sixteen hours. The following is a list of the three employers convicted, as against thirteen in the past year and live in 1869. 1. Mrs. Elizabeth Stratton, dressmaker and milliner, No. 10, Halkin-street West, June 22nd, at Westminster: Fined 10s. for each offence and costs. 12 2. Miss Mary Tandy, dressmaker, &c., No. 93, Parkstreet, Grosvenor-square, June 29th, at Marlboroughstreet: Fined 40s. and costs. 3. Madame Flavia Zoe, dressmaker, &c., No. 9, Brutonstreet, July 6th, at Marlborough-street: Fined 40s. and costs. Gas. One hundred and ninety-five observations were made during the year upon the four gases supplied by the London and Equitable Companies, at No. 73, Bessboroughstreet and Cambridge-street respectively. The gases were burned in accordance with the Metropolis Gas Act, 1860, and the illuminating power continued above the standard required by the Act. With respect to purity all the gases were free from sulphuretted hydrogen, and so far free from ammonia that they did not discolour the turmeric paper, except on April 13th, 20th, May 12th, 18th, 26th, June 3rd, 7th, 24th, July 21st, August 9th, 1870, and February 22nd and March 22nd, 1871, when the common gas of the London Company presented traces of ammonia.
